Hospice Liaison Nurse — Hospital Admissions & Outreach

Gilchrist Inova Hospice Care is seeking a Hospice Liaison Nurse to generate admissions from hospital referrals and coordinate clinical assessments for potential patients. You will cultivate relationships with hospitals, physicians and social workers while coordinating the admissions process.
