Overview
Licensed Practical Nurse (LPN) / LVN – Travel, Long Term Care, Clifton Springs, NY. This 13-week travel assignment begins 08/11/2026 and offers an evenings shift (5x8). The role provides a weekly pay range of $1,331–$1,456 with no guaranteed hours. The position is in a nursing facility/long-term care setting with a 1:10 patient ratio and includes a 4-hour orientation.

Responsibilities
- Provide skilled nursing care for adults and geriatrics in a long-term care setting
- Administer medications (including IM and SQ) and manage basic clinical treatments
- Monitor vital signs; support wound care, feeding tube and Foley catheter care as needed
- Deliver oxygen therapy and assist with activities of daily living (ADLs)
- Communicate status changes to the supervising RN/MD and follow isolation precautions/PPE protocols
- Document patient status and care in the electronic charting system (EPIC) and ensure accurate, timely updates

Requirements
- Active New York LPN/LVN license in hand (NY license is not compact)
- BLS certification
- Minimum 1 year of relevant LPN/LVN experience
- Previous charge experience required
- LTAC experience required
- Travel experience required
- Ability to float within applicable skillset
- EPIC charting experience required (hard requirement)
- Education: LPN/LVN diploma or equivalent from an approved program
