Licensed Practical Nurse (LPN/LVN) – Travel assignment in Long-Term Care
Location: Clifton Springs, NY
Unit: Skilled Nursing / Long-term Care / Nursing Home
Shift: Nights, 3x12 (1900–0730)
Start date: 08/11/2026
Duration: 13 weeks
Weekly pay: $1,355–$1,423 per week
Setting: Skilled Nursing / Long-term Care
This is a travel, temporary assignment requiring an active New York LPN license (state-specific, NY is not a compact license).

- Travel assignment with required travel experience and unit-specific nursing duties

Responsibilities
- Administer prescribed medications and treatments for adult and geriatric residents, following physician orders and facility policies.
- Monitor vital signs, manage wound and ostomy care, and support pressure ulcer prevention; perform IV starts/maintenance as allowed.
- Provide enteral feeding support and feeding-tube care; perform Foley catheter care (male and female) and oxygen therapy as needed.
- Support basic respiratory care, including BiPAP/CPAP and trach/vent care within the LPN scope; assist with pre/post hemodialysis as required.
- Document care and interventions in EPIC; coordinate with the interdisciplinary team (PT, Respiratory, Radiology, Social Services, Pharmacy) to support resident needs.
- Conduct routine diagnostics (blood glucose, 12-lead EKG, clean-catch urine collection) and respond to clinical changes; contribute to safety and infection control efforts.

Requirements
- Active New York LPN license (state-specific; NY is not compact) and BLS certification
- Minimum 1 year of LPN experience; previous charge nurse experience required
- Travel experience required; first-time travelers accepted
- EPIC charting experience required
- Unit/setting experience in Skilled Nursing / Long-Term Care; geriatric patient population
- Ability to float within scope of practice as needed
