Weekly pay: $1,386–$1,401 per week
Shift: 5x8 Evenings
Setting: Nursing facility / nursing home
This assignment is a travel, temporary role within a long-term care environment serving adult and geriatric residents.

- Setting: Nursing facility / nursing home with a 20-bed census
- Scheduling: Every other weekend; every other holiday (if needed); 5x8 with potential floating within skillset
- RTO policy: client may cancel up to 3 shifts during the contract without charges
- Orientation: 4 hours prior to start

Responsibilities
- Deliver direct patient care to adult and geriatric residents in a LTAC/Skilled Nursing setting, including medication administration (IM and SQ), vital signs, admissions, and ongoing monitoring
- Perform skilled nursing procedures: feeding tube care (G-tube/NG), Foley catheter care (male and female), and oxygen therapy; implement isolation precautions and PPE use
- Maintain accurate, timely documentation in EPIC; participate in care planning and coordinate with interdisciplinary team (Respiratory, Radiology, Social Services, Pharmacy)
- Monitor for clinical changes, respond appropriately, escalate to the charge nurse as needed, and participate in patient transfers and admissions
- Support a safe, patient-centered environment with appropriate infection control, fall prevention, and activity of daily living (ADL) assistance

Requirements
- Active New York LPN license (in-hand; pending licenses not accepted)
- BLS certification
- Minimum 1 year of recent LTAC or skilled nursing experience; previous charge experience required; travel experience preferred
- Experience working with adult and geriatric populations; LTAC/Skilled Nursing background strongly preferred
- EPIC charting experience required (proficiency confirmed on submission)
- Ability to float within scope of practice and to work the evening shift on a 20-bed unit with 1:10 patient ratio
