Recovery Counselor — Community-Based Support (Sign-On)
Trilogy
Trilogy is seeking a Recovery Counselor in Chicago, IL to assess clients’ personal, medical, emotional, social, and environmental needs and plan treatment through home, community, and office visits. You will provide strengths-based, trauma-informed services and connect clients with community resources using a harm-reduction approach. The role involves documenting services, completing case management tasks, and providing crisis/on-call coverage while maintaining 75% community-based work in the #J-18808-Ljbffr Trilogy
