The ideal candidate must have LTC experience and strong assessment and communication skills to evaluate patient needs, establish appropriate treatment plans, and help patients achieve their maximum functional potential.\n \n Responsibilities\n \n Evaluate patients and assess speech, language, communication, and swallowing needs. \n Develop and implement individualized treatment plans based on patient needs and clinical findings.\n Perform ongoing assessments and monitor patient progress.\n Establish accurate prognoses and guide patients toward achieving their maximum potential.\n Communicate effectively with patients, medical staff, and facility personnel.\n Maintain accurate and timely clinical documentation.\n Collaborate with the interdisciplinary healthcare team to support patient care.\n Follow facility policies, clinical standards, and applicable regulatory requirements.\n Utilize EMR systems and Microsoft Office for documentation and communication.\n Apply knowledge of the PDPM reimbursement system in the long-term care setting.\n \n Required Qualifications:\n \n Graduate of a college or university accredited by the American Speech-Language-Hearing Association (ASHA) with a degree in Speech-Language Pathology.\n Current New York State Speech-Language Pathologist license.\n Eligible for or holds the Certificate of Clinical Competence (CCC-SLP) from ASHA.\n LTC experience is required.\n Excellent communication skills.\n Strong clinical assessment and evaluation skills.\n Ability to accurately determine patient prognosis and treatment potential.\n Proficiency with Microsoft Office and Epic/EMR software.\n Knowledge of the PDPM reimbursement system.\n \n Travel / Local Requirements:\n \n 75-mile radius rule applies to travelers.\n Local candidates are accepted.\n Candidates living within 75 miles will be considered at the local rate.\n \n Key Skills\n \n Long-Term Care (LTC)\n Speech and language evaluation\n Patient assessment\n Treatment planning\n Clinical documentation\n Patient communication\n Interdisciplinary collaboration\n Epic/EMR \n Microsoft Office\n PDPM reimbursement system\n Patient-centered care\n \n \n About Talent4Health\n \n A leader and innovator, offering comprehensive talent solutions for the healthcare industry across the nation.
