Our PurposeInspired Home Health is dedicated to providing compassionate and personalized in-home health care, ensuring the health, comfort, and independence of our clients in the comfort of their own homes.Role OverviewAs a Home Health RN East Valley, you provide skilled in-home nursing that supports safe recovery and improved outcomes following hospital, rehab, long-term care, or SNF discharge. You will deliver care aligned to the established plan of care, collaborate with the interdisciplinary team, and maintain accurate, timely documentation while safeguarding PHI.Key responsibilities include completing in-home skilled visits; assessing patient status; delivering ordered treatments; providing patient and caregiver education tied to diagnosis and care plan; coordinating services and communicating needs for other disciplines (HHA, OT, PT, MSW, ST, and paraprofessionals); and partnering with the Clinical Manager for ongoing performance and development.Required qualifications/skills: - Active RN license- Current CPR certification- Current TB test- Fingerprint Clearance Card- Valid driver’s license, reliable personal vehicle, and active auto insurance- Strong clinical judgment, communication, time management, and compassionate customer serviceIf you value one-on-one care in the home setting, submit your resume today.Typical WorkdayYour weekday schedule (Monday–Friday) starts by reviewing your visit plan and confirming timing with patients and caregivers. You travel throughout the East Valley to see clients in their homes, focusing on clear, respectful communication and a calm, professional presence. Between visits, you document in the point-of-care EMR and return timely calls to keep patients, families, and partners informed.
