Pay $85,000 - $100,000 per year Scheduling Flexible scheduling, management of caseload and coverage territories Location Multiple Areas Available in East Valley Phoenix including Tempe, Mesa, Gilbert and Chandler Position Home Health Registered Nurse RN Job Description We are seeking a skilled Registered Nurse who will be a Nurse Case Manager responsible for improving a patient’s quality of life by planning, directing, and providing home care services. The RN will provide direct nursing care and manage a team of therapists and other professionals to treat patients under a physician’s plan of care, following agency policies and the State Nurse Practice Act. The RN’s goals are to rehabilitate and educate patients to regain and retain independence in their home setting. The care plan will include clinical care, symptom control, and identification of physical, psychological, social, and spiritual needs. Key Duties Follow Agency Policies and Procedures, Federal and State Regulations and Standards of Practice. Attend Agency In‑services, Case Conference, or other meetings as requested. Provide skilled nursing services in accordance with the orders of a physician and under a plan of care in a patient’s home. Be responsible for the observation, assessment, nursing diagnosis, counseling, care and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others. Record the health needs of the patient. Perform and complete start‑of‑care assessment, medication profile, initiate Nursing or Home Health Aide plan of care, and/or Homemaker/Companion service plan and associated paperwork in accordance with agency policy. Evaluate the effects of the care given and regularly reevaluate the patient’s nursing needs. Evaluate and revise the plan of care as necessary. Provide those services requiring substantial specialized nursing skill based on the physician’s orders and the nursing diagnosis, such as physical assessment, patient and family teaching, urinary catheter insertion. Requirements Currently licensed as a Registered Nurse to practice in the State of Arizona (or) a current multi‑state license from another compact state with a valid compact license for up to 30 days until an Arizona permanent license is issued. 1 year experience in Nursing (preferred). Associate degree (preferred). Willingness to travel 25% of the time (preferred).
