Description

• Conduct evaluations for patients in either inpatient or outpatient environments and craft personalized treatment plans
• Implement physical therapy interventions emphasizing mobility, strength, balance, and functional recovery
• Collaborate with nurses, physicians, case managers, and other rehabilitation specialists to ensure optimal results
• Provide education to patients and their families regarding therapy goals, plans for discharge, and home exercises
• Maintain thorough documentation of evaluations, treatments, and patient progress in line with facility protocols and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to guarantee cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from a recognized institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
