Diabetes Educator - Anderson Diabetes Clinic

Position Overview The diabetes educator provides an educational experience for patients and their significant others to promote effective diabetes self-management. The role is multidisciplinary and involves assessing patient strengths and educational needs, collaborating with the care team to develop individualized plans, incorporating technology and knowledge based on ADA, AADE, and regulatory standards, promoting safe diabetes self-management practices, empowering patients with self-care skills, consulting with healthcare professionals and organizations, and performing related tasks as needed. The position is located in Anderson, Indiana and will consider a Registered Nurse or Registered Dietician. Responsibilities Assess patient strengths/educational needs using available data and resources. Collaborate with the team to establish/prioritize individualized plans with patients and significant others. Incorporate generally accepted technology and knowledge into patient education experience based on standards of practice for own discipline, ADA, AADE and other regulatory agencies. Promote safe practice of diabetes self-management related to medication administration, meal planning, exercise, blood glucose monitoring and other aspects of care. Empower patients/significant others with knowledge and skills in self-care for optimal health and psychosocial adaptation, to maintain/improve quality of life. Consult with other health care professionals and organizations to utilize appropriate resources (human, financial and material) which promote consistency and continuity of care of high value. Perform other related tasks as needed. Qualifications Graduate of a National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana (Nurse Licensure Compact) or licensed as a Registered Dietician in Indiana (LD) and Registered Dietician Certification (CDR) required. Bachelor’s Degree required. Certified as a diabetes educator required. Two years or more of Registered Nurse or Registered Dietician experience required. One to two years of educating patients in the area of diabetes preferred. Excellent communication, team, compassionate, caring, accountable, and self-directed skills.
