Job Title Oak Street Health New Graduate Nurse Practitioner Fellowship Location City Line – 1114 Liberty Ave, Brooklyn, NY 11208 Program Overview Oak Street Health’s 12‑month paid Nurse Practitioner Fellowship in Adult Primary Care, in collaboration with the University of Michigan School of Nursing, focuses on a value‑based care model for older adults. Fellows build patient panels, receive training from preceptors, and complete didactic courses designed to build on graduate training. Roles & Responsibilities Develop and manage a patient panel of older adults in a primary care setting. Provide direct patient care under supervision of experienced clinicians. Participate in didactic coursework and clinical learning experiences. Collaborate with an interprofessional team to deliver high‑quality care. Commit to employment with Oak Street Health for at least one year following completion of the fellowship. Qualifications Family, adult, or adult‑gerontology primary care nurse practitioner certification. Graduated or slated to graduate between December 2024 and May 2026. Must be newly licensed as an NP or in the process of obtaining board certification by 6/6/2026. Minimum 1year of experience as an adult RN within the last five years. Minimum GPA of 3.0 in NP program. State NP license active, non‑probationary by 8/3/2026. Fluency in Spanish strongly preferred; bilingual Spanish required for this opening. Strong passion for adult primary care with older adults and willingness to relocate to the program location. Demonstrated teamwork, commitment to high‑quality care, and alignment with Oak Street values. Benefits & Compensation Annual salary of $112,000 (location specific).
