Responsibilities

• Provide direct patient care, including vital sign monitoring, hygiene assistance, and mobility support
• Assist patients with activities of daily living (ADLs) such as bathing, dressing, and grooming
• Take vital signs, report changes in patient condition to nursing staff, and maintain accurate records
• Collaborate with healthcare team members to develop and implement individualized care plans
• Maintain a clean and safe environment for patients, including preparing and cleaning equipment, supplies, and areas
• Maintain confidentiality and adhere to HIPAA standards
• Demonstrate a commitment to patient-centered care and a willingness to adapt to changing circumstances

Requirements

• High school diploma or equivalent required

• Current CNA certification in the state of Illinois

• Strong communication and interpersonal skills

• Ability to lift, transfer, and reposition patients as needed

• Basic life support (BLS) certification preferred
