HomeCentris Home Health is seeking an Occupational Therapist (OT) to provide quality care and occupational therapy modalities to its Medicare skilled homecare services in the setting where our clients most want to be, in their homes.

Duties of occupational therapist:

• Performs accurate and comprehensive assessments so patients are safe and happy.
• Obtains physician orders for certification and recertification of home occupational therapy services
• Coordinates care planning and delivery with the physician and other healthcare team members and agency staff to facilitate optimal patient outcomes.
• Utilizes appropriate pain management modalities that include, but are not limited to, occupational therapy modalities.
• Other duties as assigned.

Requirements:

• Current Occupational Therapy license.
• Two years of skilled home health experience.
• Three years of healthcare experience.
• Four years of Occupational Therapy experience.
