Acute Care Case Manager

This is a 13-week contract position.

Day shift, 5x8 hours (08:00 - 16:30). Must be available to work every other weekend.

We are seeking an experienced Acute Care Case Manager to join our team. The ideal candidate will have a strong background in acute care case management and discharge planning, with a proven ability to coordinate patient care effectively.

Responsibilities:

• Manage the care coordination and discharge planning for a caseload of up to 25 patients.
• Participate in family meetings and collaborate with interdisciplinary teams.
• Assess patient needs and develop comprehensive treatment plans.
• Address issues related to insurance, prior authorizations, and eligibility for services.
• Ensure compliance with CMS guidelines, Joint Commission standards, and other relevant regulations.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of acute care case management or discharge planning experience.
• Hospital experience is mandatory; behavioral health experience is not applicable.
• Current CA state nursing license required (pending licenses accepted if licensed upon start date).
• Experience with Epic charting system preferred.
