Day shift, Monday to Friday from 8:00 AM to 4:30 PM.

Job Description

We are seeking a dedicated Physical Therapist to join our home health team in Gassaway, West Virginia. This role involves providing high-quality care to patients in the home health setting, focusing on general orthopedic cases, generalized weakness, strengthening, and cardiac rehabilitation. The therapist will work within an assigned coverage area and will be responsible for evaluating patients, developing treatment plans, and implementing therapy interventions.

Responsibilities

• Evaluate patients and develop customized treatment plans.
• Provide physical therapy interventions in a home health setting.
• Maintain accurate documentation using the Epic charting system.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Manage a caseload effectively, seeing 5-6 patients per day.

Required Experience / Certifications / Licensure

• Valid Physical Therapist license in the state of West Virginia.
• BLS certification required.
• Experience in home health care or outpatient therapy preferred.
• Strong interpersonal skills and a positive attitude towards learning and patient care.
