PT - Physical Therapist

The Physical Therapist will provide home health services to patients in various counties. The therapist will have a manageable caseload of 5-6 patients per day, focusing on a range of diagnoses including general ortho issues, generalized weakness, strengthening, and cardiac rehab. Responsibilities Conduct initial assessments and develop individualized treatment plans. Provide ongoing therapy and progress evaluations for patients in a home setting. Collaborate with other healthcare professionals to ensure comprehensive patient care. Utilize the Epic system for documentation and patient management. Participate in staff meetings and professional development activities. Required Experience / Certifications / Licensure Valid Physical Therapist license to practice in West Virginia.
