Communities that Care Program Coordinator Willmar, MN, USA Job Type: Full Time Salary Range: $51,929 About the Role JOB TITLE: Program Director/Coordinator TITLE OF IMMEDIATE SUPERVISOR: PACT for Families Executive Director JOB SUMMARY: The CTC Program Coordinator will lead the implementation of the Communities That Care (CTC) framework across multiple school districts and county coalitions within the PACT for Families Collaborative region. This individual will serve as the primary point of contact for the Department of Human Services (DHS) regarding CTC activities and will oversee cross-county alignment of prevention strategies to reduce youth substance use by strengthening protective factors and building community cohesion. The ideal candidate will have strong community facilitation skills, the ability to build and maintain relationships across sectors, and a solid foundation in public health or prevention frameworks. This position is central to ensuring fidelity to the CTC model and sustainability of prevention efforts beyond the grant period. Specific Responsibilities Lead the coordination and development of the Communities That Care (CTC) initiative in partnership with county public health agencies and school districts across the five-county service area. Support county partners in forming leadership teams and applying the Strategic Prevention Framework (SPF) and CTC model effectively. Provide training and technical assistance to coalition staff and partners using a train-the-trainer model to build long-term local capacity in the CTC framework. Facilitate the engagement of community stakeholders—including school personnel, families, and youth—in prevention planning and CTC Community Board activities. Coordinate the administration and completion of data collection tools to support the SPF and CTC planning processes. Serve as the primary point of contact with the Minnesota Department of Health (MDH) grant manager and coordinate related consultant visits, communications, and meeting logistics. Collaborate with other PACT for Families Collaborative programs to ensure alignment and integration of prevention efforts. Qualifications, Skills, and Experience Required Bachelor’s degree in Public Health, Education, Social Work, Community Psychology, or a related field, or equivalent professional experience. A minimum of five years of experience in a related role is preferred. Experience in coordinating services, developing projects, and working in partnership with multiple agencies. Background in chemical health prevention is strongly preferred. Demonstrated knowledge of human services systems, educational and mental health services, and collaborative service delivery models. Strong organizational and leadership skills, with proven ability to lead teams and manage projects. Excellent written and verbal communication skills, including the ability to prepare and present complex information clearly. Effective facilitation, training, and interpersonal skills; able to foster consensus in diverse and collaborative settings. Experience working with diverse populations and a strong commitment to equity and inclusion. Ability to work independently and as part of a team in dynamic, cross-sector environments. High level of initiative, creativity, and problem‑solving ability. Demonstrated cultural sensitivity and the ability to build trusting relationships with stakeholders from a variety of backgrounds. Travel There is significant travel within the PACT service area, occasional travel in the State. Valid driver's license, car insurance and vehicle required. Work hours may require late afternoon and evening meeting times and appointments. Hours Worked and Salary Range This is a full-time, salaried, benefit position.
