Schedule and Territory Schedule: Saturday and Sunday and 1 weekday 8:00am-5:00pm. On-call requirements: 2 weekdays per month, 1 summer and 1 winter holiday. Territory: Center Line, Hazel Park, Warren, Ferndale, Royal Oak, Detroit, River Rouge, Hamtramck areas. Sign-on bonus: 10,000.00. Responsibilities Provide home-based nursing care for patients as directed by an attending physician, with one-on-one patient care and care at the top of the license. Coordinate care as part of an interdisciplinary team to support positive patient outcomes. Interact with patients’ families and participate in the patient recovery journey. Plan of Care: Observe and monitor patient conditions and perform OASIS assessments to develop an individualized care plan and adjust as needs change. Minimize patient risk: administer medications as prescribed and prioritize visits for high-risk patients to reduce rehospitalizations. Continuity of Care: Manage multidisciplinary care and ensure appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate patients and families on disease processes using teach-back methods to ensure understanding. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines; collaborate with the interdisciplinary team and physicians to ensure optimal care. Rules and Regulations: understand and follow agency policies, procedures, rules, and regulations; communicate changes in schedule/availability to schedulers or supervisors. Operations: Attend in-service trainings and mandatory agency meetings. Qualifications Registered Nurse with current license in the state of employment. Minimum one-year experience as an RN in an acute care setting. Home care experience preferred. Valid driver’s license and auto insurance in the applicant’s name as a driver. Capable of performing all physical demands.
