Job Description
Title: Physical Therapist - Skilled Nursing

Schedule:
Shift: 08:00:00 - 16:30:00
Days: Monday - Friday

Assignment Details:
Guaranteed Hours: 40
Contract Length: 52 weeks

Requirements:
- Relevant state licensure and certification as required for the role
- Experience in a skilled nursing facility is preferred

Roles & Responsibilities:
- Provide direct physical therapy services to patients in accordance with their individualized treatment plans.
- Assess patient needs and develop appropriate rehabilitation goals.
- Collaborate with a multidisciplinary team to ensure optimal patient care.
- Document patient progress and modify treatment plans as necessary.
- Educate patients and their families on exercise programs and home care.
- Maintain a professional and compassionate approach while working with patients.

Compliance / Notes:
- 10% salary placement fee applies.
- Candidates cannot take new assignments at this time.
- All disciplines are welcome (PT, OT, SLP, PTA, OTA).
