As an Evening Shift LPN, you will play a vital role in creating a warm and inclusive environment for our residents. We are seeking a compassionate, skilled, and dedicated LPN to join our dynamic team. Responsibilities: Provide direct care to residents, including administering medications, performing vital signs, and assisting with daily living activities Develop and maintain personalized care plans, working closely with the care team to ensure resident needs are met Assist with mealtime, including serving, feeding, and ensuring hydration needs are met Participate in care conferences and contribute to resident assessments, care plans, and service plans Maintain accurate and timely records of resident care and services Collaborate with other healthcare professionals, including physicians, CNAs, and staff members to ensure high-quality care Engage with residents, families, and staff members to promote a positive and inclusive environment Perform other duties as assigned by the shift leader or director of nursing About the Role: As an Evening Shift LPN at Willow Springs Memory Care, you will have the opportunity to make a meaningful difference in the lives of our residents.
