Care Manager

Experience with Care Coordination is required. Utilization review experience is necessary. Discharge planning experience is a must. Computer literacy is essential. A minimum of five years of clinical experience as a Registered Nurse is required. A Bachelor's or Master's degree in Nursing is preferred. Candidates must hold a valid RN license in the State of New York. PRI and Screen certification must be obtained within three months of employment. Local candidates are preferred. Leadership experience is an advantage.
