Hospice Registered Nurse Case Manager

Role: Hospice Registered Nurse Case Manager Responsibilities Provide compassionate end-of-life care in patients' homes Manage a caseload of hospice patients, conducting comprehensive assessments Develop and update individualized care plans Administer skilled nursing care with a focus on pain and symptom management Coordinate with physicians, social workers, chaplains, and aides to ensure seamless care Ensure regulatory compliance and maintain high-quality documentation Educate and support patients and their families Participate in on-call rotations as needed Qualifications & Skills Current RN licensure Strong clinical assessment and care planning skills Excellent communication and collaboration abilities Ability to provide emotional support and education to patients and families Knowledge of hospice regulations and documentation standards Compassionate, respectful, and mission-driven mindset #J-18808-Ljbffr
