Hospice Registered Nurse Case Manager

Role: Hospice RN Case Manager Manage a caseload of hospice patients, providing compassionate, in-home end-of-life care. Conduct comprehensive assessments and develop individualized care plans that prioritize comfort, dignity, and family needs. Collaborate with social workers, aides, chaplains, and physicians to coordinate services and ensure high-quality, compliant care. Manage pain and symptoms effectively to enhance patient comfort.
