Position Summary $21.98 - $24.73 / hour Position Summary: Case Aides report to the FFT-CW Supervisors. They provide culturally competent case management and advocacy to families in the FFT-CW Program in compliance with ACS policies and procedures. The Case Aide is part of a clinical team that comprises an FFT CW Program Director, a Supervisor, and up to four Therapists or Interventionists. The Case Aides' duties are performed in support of, as well as in coordination and collaboration with the FFT-CW Therapists, Interventionists, and Supervisors. Their primary duties are to assist and support the case planners in collecting critical information, such as educational and medical data, and linking families to appropriate community resources. Based on case planners' assessments, the Case Aide will provide families with concrete case management support and advocacy services to address needs in areas such as public assistance, housing, school-related issues, and referrals for community-based services such as parenting classes, mental health services, job readiness programs, after school activities, summer camps, and other pro‑social activities. FFT-CW (Child Welfare) is a therapeutic program in our Prevention continuum of services. It is a home‑based intensive therapeutic model for families in the Bronx with multiple risk factors, where there has been physical abuse and/or neglect of a child/ren aged 0‑17. It is an evidence‑based family therapy model that focuses on engaging with the family's broader social environment and develops each family's social and community networks. Service Tracks Low Risk track supports families that are more likely to successfully engage in community-based interventions and programs. High Risk track supports families that need intensive change processes delivered in the home to address the risk of placement or where there are issues of child safety, neglect, family violence, mental health concerns, substance abuse or at risk of involvement in the criminal justice system. Essential Duties Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures. Assist with concrete case management tasks, e.g., rent arrears, utility shut off, furniture assistance. Perform system updates on PROMIS and CONNX. Assist with intake tasks: set up intake packets, open, and close case records. Monitor school attendance and obtain school records. Request medical records for all minor children. Assist with the maintenance of case records for the FFT CW Program. Conduct "Safe Sleep" assessments of infants as well as ongoing safety and risk assessments of younger children in the home as needed. Assist with FTCs (Family Team Conferences) by helping clients get to and from the conferences and sending out letters to the families. Conduct assessment of financial resources for families as needed. Research and identify appropriate community-based services to support families, e.g., mental health programs, DV programs, pro‑social activities for adolescents, etc. Accompany clients to entitlement appointments such as HRA/Public Assistance. Make referrals/appointments to agencies/collaterals needed. Complete all required child welfare documentation such as progress notes, assessment forms, etc., for activities performed. Assist with coverage of the front desk as needed. Answer the telephone, screen and direct calls while covering the front desk. Gather resources for clients (e.g., camps, vocational/alternative school settings, housing info, recreational activities). Collect Safe Mothers, Safe Children (SMSC) screening forms, Domestic Violence (DV) screening forms, and Health Connections (HC) screening forms, and make referrals to Health Connections, SMSC, and Family Wellness if appropriate. Attend all trainings necessary to perform the above tasks. Perform other duties as assigned. Minimum Qualifications Associate degree required; bachelor's degree in social work (BSW) or a BA in a related field preferred. Minimum one year of related experience working with children, adolescents, and/or families preferably in a child welfare, juvenile justice, or mental health settings. Key Competencies Bilingual in Spanish/English speaking preferred. Ability and willingness to work with families and teens in the home, school, and other appropriate settings in the community. Ability to work flexible hours including evenings and some weekends as needed. Ability to work independently, as well as contribute to a team. Very good interpersonal/communication skills, including the ability to write well and complete documentation in a timely manner. Computer literate. Schedule The incumbent is expected to work a schedule, whether in‑person or hybrid, as determined by the department's needs to facilitate effective collaboration with the team. This flexibility is essential for maintaining seamless communication, fostering teamwork, and ensuring the smooth operation of the department's processes. The position's requirements are subject to periodic review and adjustment based on organizational needs and changes in work dynamics.
