Key Responsibilities

• Evaluate and treat patients with speech, language, cognitive, and swallowing disorders in a skilled nursing facility setting
• Develop and implement individualized treatment plans based on patient evaluation findings
• Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and their families on therapeutic techniques and home exercise programs

Required Qualifications

• Master's degree or higher in Speech-Language Pathology from an accredited program
• Active and unrestricted Colorado SLP license (or eligibility to obtain)
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA, or Clinical Fellowship eligibility
• Experience working in a skilled nursing facility or similar clinical environment

Preferred Experience / Skills

• Prior experience in a SNF or long-term care setting
• Proficiency with dysphagia assessment and treatment, including modified barium swallow studies
• Familiarity with MDS documentation and Medicare/Medicaid billing requirements
