Job Overview – Clinical Outreach RN Compensation: $90,000 – $105,000/year + bonus Location: Penobscot County, ME Schedule: Monday to Friday, 9:00 AM – 5:00 PM TAG MedStaffing is hiring a Clinical Outreach RN in Penobscot County, ME for our client, supporting referral development, patient transition coordination, and clinical relationship management within a growing home infusion environment. This role will work closely with hospitals, managed care teams, physicians, discharge planners, and healthcare providers to identify patients, coordinate care plans, and facilitate smooth transitions from acute care settings to home-based infusion services. The ideal candidate is a Registered Nurse with strong clinical knowledge, relationship‑building skills, and an interest in business development or healthcare sales. Responsibilities as the Clinical Outreach RN Referral Development: Build and maintain relationships with hospitals, managed care organizations, physicians, discharge planners, and healthcare providers to generate patient referrals. Care Coordination: Collaborate with clinical teams, providers, and patients to coordinate care plans and support smooth transitions from hospital to home infusion services. Clinical Education: Educate healthcare professionals, patients, and caregivers on infusion services, treatment options, and patient eligibility requirements. Business Development: Support territory growth initiatives, relationship management activities, and ongoing referral development efforts within assigned healthcare networks. Patient Advocacy: Serve as a clinical resource and patient advocate throughout the intake, transition, and care coordination process. Cross-Functional Collaboration: Partner with pharmacy, nursing, reimbursement, intake, and operational teams to ensure timely and effective patient service delivery. Qualifications for the Clinical Outreach RN Education: High school diploma or equivalent is required, while an Associate’s or Bachelor’s degree in Nursing is preferred. Licensure: Current and unrestricted Registered Nurse (RN) license is required. Experience: 2+ years of infusion nursing, infusion healthcare, home infusion, hospital nursing, or related clinical experience is required. Industry Knowledge: Experience working with managed care teams, discharge planning, care coordination, or infusion services is preferred. Technical Skills: Proficiency with Microsoft Office Suite, electronic medical records, and healthcare documentation systems is preferred.
