Job Summary : Serve Ingleside Memory Support Assisted Living residents by supporting each person’s best abilities and capabilities toward thriving. Plan and implement group and individual life enrichment programs to maximize social, physical, emotional, occupational, educational, and cognitive functioning. Responsibilities Assist the Director of Memory Support and Programming with planning and executing person‑centered, individualized programs. Plan and lead individual and group activities in the Memory Support Assisted Living neighborhood. Develop, promote, and implement life enrichment programming encompassing the Seven Dimensions of Wellness. Assist with transportation of residents between program locations as needed. Assess residents’ needs, interests, and strengths through interviews and collaboration with family members; create individualized programming. Complete and monitor assessments, care plans, progress notes, and other documentation in compliance with facility requirements. Participate in meetings such as Care Plan meetings, Huddles, and Program Planning meetings. Conduct Engaged Living groups and one‑to‑one activities in a meaningful, person‑directed manner. Motivate residents to participate in chosen activities. Evaluate programs and discuss improvements with the Director of Memory Support and Programming. Adapt activities to meet residents’ needs. Provide one‑to‑one support as indicated by the resident care plan. Communicate information with residents, family members, and staff to create and sustain rapport. Provide oversight, coordination, training, and supervision to students, interns, and volunteers as needed. Participate in off‑site trips and follow off‑premise policies. Maintain department facilities, storage space, and equipment; assist with program set‑up and clean‑up. Communicate scheduled activities to residents, families, and staff; ensure each resident receives a copy of the monthly Life Enrichment Calendar. Plan and implement programming to cultivate community on the Neighborhood. Integrate understanding of residents’ developmental, mental, social, and cultural needs into programming. Assist in dining room as needed. Work during holidays and special events as instructed. Provide education and support to family members as needed. Perform other duties as required. Qualifications High School diploma or Associate’s Degree in Human Services or related field. Six (6) months to one year of related experience, or equivalent combination of education and experience. Certification as a Dementia Practitioner (CDP) or eligibility to obtain CDP within 90 days of hire. Strong verbal and written communication skills. Strong interpersonal and organizational skills. Proficiency in computer applications such as Microsoft Office. Ability to apply common sense in following instructions, both written and oral. Experience with Adult Day programming is a plus.
