Hospital Social Worker - Discharge & Crisis Support

Trinity Health

MercyOne Health System is seeking a dedicated Social Worker for MercyOne Des Moines with a 72-hour biweekly schedule across a 12-hour day shift. The role focuses on psychosocial evaluation, discharge planning, and coordination of community resources to support patients and families during transitions in care.
