Case Manager for 55+ Senior Housing — Flexible Hours
CAMBA
CAMBA seeks a Case Manager to support its Simba Simbi Supportive Housing Program in Brooklyn. You will review eligibility, create tenant files, and conduct intakes and ongoing assessments of tenants and families. You will collaborate to revise living plans and connect tenants with community resources and services. You will help tenants progress toward goals, advocate for self-sufficiency, and document progress for reporting, with occasional evening coverage as needed. #J-18808-Ljbffr CAMBA
