Job Summary The Critical Care Clinical Specialist is responsible for ensuring safe, effective, and cost‑effective drug therapy for patients in critical care areas. This role involves close communication with healthcare professionals and patients to monitor pharmacotherapy outcomes and provide clinical pharmacy services. Duties and Responsibilities Participate in daily patient care rounds and monitor drug therapy for appropriateness, dosage, route, and interactions; discuss medication clarifications with prescribers and document all changes. Maintain and update patient profiles with demographics, allergies, and current medications daily; promote formulary use and coordinate procurement for non‑formulary drugs. Provide competent care for adult, pediatric, geriatric, adolescent, and psychiatric patients as appropriate for age. Inspect pharmacy records to monitor drug therapy for safety, detect and report adverse drug reactions and medication errors. Perform pharmacokinetics, IV to PO conversions, therapeutic interchanges, and other clinical pharmacy functions promptly and accurately. Promote safe, efficacious, and cost‑effective medication therapy through documentation of clinical services and medication utilization initiatives. Serve as a consultant to Pharmacy & Therapeutics committee and other performance‑improvement committees. Participate in clinical rounding, public speaking, and presentation of clinical and financial data; create educational material for pharmacy staff and other professionals. Plan and implement system‑wide pharmaceutical care programs; complete one clinical project annually in collaboration with the pharmacy clinical team. Ensure all patient care meets or exceeds all regulations, statutes, and HFAP standards. Report daily to Clinical Pharmacy Manager on operations, projects, and role expectations. Respond to medical emergencies as appropriate. Counsel patients on discharge medications and coordinate medication distribution with outpatient pharmacy when needed. Work rotating AM & PM shifts. Job Specifications Knowledge Pharm.D. or equivalent from an ACPE‑accredited School of Pharmacy with a current pharmacist license in good standing. Critical Care PGY1/PGY2 residency recommended; at least 3 years of relevant clinical experience preferred. Board Certification recommended. Proficiency with computer applications and pharmacy information systems. Thorough knowledge of state and federal pharmacy laws and regulations; proficiency with new medications and technology. Effective verbal and written communication skills; ability to collaborate on interdisciplinary teams or committees. Knowledge of pathophysiology, diagnostic criteria interpretation, pharmacokinetics, and pharmacotherapeutics; strong analytical and problem‑solving skills. Skills Ability to work independently, manage stress, handle multiple tasks, prioritize, and adapt to workload changes. Team‑building skills and effective communication (verbal and written). Abilities Ability to work independently and cooperatively with other members of Insight Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and respond to client concerns.
