Job Description Under the direction of the Ambulatory Care Manager, the Ambulatory Care Supervisor oversees the daily clinical operations and patient care activities of assigned care teams. The role balances direct patient care and supervisory duties to ensure the highest quality service and care are provided. The Supervisor is responsible for delivering patient‑centered services in accordance with the San Ysidro Health Model of Care, which aims to provide safe, high‑quality, cost‑effective care and improve health outcomes. Essential Functions of the Job Supervise and coordinate daily team‑based functions according to the Patient‑Centered Medical Home care model. Lead care teams in achieving organizational goals for health outcomes improvement, patient safety, satisfaction, and clinical quality. Follow evidence‑based guidelines to develop department protocols, policies, and procedures that optimize clinical outcomes. Implement and monitor approved medical management protocols and pathways. Select, train, orient, supervise, and develop nurses and medical assistants; evaluate clinical competencies and complete performance reviews. Maintain optimal staffing schedules and provide direct patient care as needed to support clinic operations. Supervise assigned care team staff, oversee operations, and build teams that meet community needs. Ensure staff complies with the Nurse Practice Act, TJC, federal/state/local, and health plan requirements. Maintain continuous audit readiness by conducting rounds and ensuring daily compliance with guidelines. Use effective problem‑solving techniques and collaborate with Clinical Services Manager to provide disciplinary action as needed. Ensure clinical preparedness to meet patient care needs by overseeing initiatives such as infection control, medication, immunization, and administration compliance. Serve as the nursing representative at Continuous Quality Improvement meetings, identifying, coordinating, and implementing workflow improvements. Additional Duties and Responsibilities Maintain professional growth and development of the department through seminars, workshops, and professional affiliations. Utilize and organize time efficiently to maximize productivity. Work independently and in a team‑oriented, collaborative environment. Adapt to shifting priorities, demands, and timelines. Attend and participate in meetings as required. Travel to multiple sites within the SYHealth network as needed. Supervise immunizations to assure compliance with program requirements. Perform other related duties as assigned or requested. Job Requirements Experience Required 1 year of clinical nursing experience as an RN, with direct reports and/or unit oversight. Education Preferred Bachelor’s degree in nursing or health care related field. Certifications Required Active California RN License and BLS certification. Verbal and Written Skills Required to Perform the Job No specific skills listed. Technical Knowledge and Skills Required to Perform the Job Knowledge of current nursing practices. Strong interpersonal and leadership skills. Proficiency in Microsoft Office and excellent mathematical computational skills. Equipment Used Office equipment such as computers, copiers, telephones, and fax machines. Medical equipment including emergency equipment, defibrillator (AED), and nebulizer. Working Conditions and Physical Requirements Frequent prolonged standing/walking. Occasional lifting, positioning, pushing, and/or transferring patients and supplies. Handle emergencies or crises. Multiple calls and inquiries at times. Exposure to unpleasant elements such as accidents, injuries, and illness. Work a varied, 40‑hour per week schedule that may include evenings and/or weekends between 8 am and 8 pm. Reliable transportation required for occasional travels to clinics in San Diego County.
