Fundamentals is seeking a compassionate, organized, and dependable Case Manager to join our team. The Case Manager plays an important role in supporting children, individuals, and families by helping them navigate services, connect with resources, and achieve their personal goals.

This position is ideal for someone who enjoys building relationships, advocating for others, coordinating services, and working collaboratively with families and community partners.

What You’ll Do:

As a Case Manager, you will:

• Provide ongoing case management and service coordination to children, individuals, and families.
• Gather information from individuals, families, schools, medical providers, and community agencies to identify needs and available resources.
• Support individuals and families using a person-centered, strengths-based, and culturally responsive approach .
• Help families understand available programs, resources, and services and assist them in making informed choices based on their needs and preferences.
• Screen referrals for children with disabilities and assist with connecting families to appropriate services.
• Develop and maintain positive working relationships with human service, medical, educational, and community organizations.
• Coordinate communication between families, service providers, agencies, and other professionals involved in an individual's care.
• Identify gaps or barriers in service delivery and participate in initiatives designed to improve services and outcomes.
• Provide education, support, and technical assistance to individuals, families, providers, and community agencies.
• Attend team, case, agency, and other required meetings.
• Maintain accurate and confidential case records and documentation.
• Prepare reports and other documentation related to case management and service delivery.
• Maintain documentation required for court proceedings when applicable.
• Complete all required documentation accurately and within three business days .
• Maintain regular attendance and participate in overtime or additional hours when needed.
• Perform other duties and responsibilities as assigned.

What We’re Looking For:

Education & Experience

• Associate's or Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred.
• Social Work certification or credential is desirable.
• Two or more years of experience in health, human services, case management, or a related field preferred.
• Experience working with children, individuals with disabilities, and/or families is a plus.
• Must possess a valid driver's license and have the ability to travel between locations and appointments as needed.

• Proficiency with computers, office equipment, and standard office software.
• The ability to travel efficiently and manage a schedule that may involve multiple locations.
• A willingness to learn, problem-solve, and advocate for the needs of the individuals and families we serve.
