It's rewarding to be on a team of people that truly believe in making an impact!We are committed to building the best primary care environment for patients and are seeking healthcare enthusiasts to join us.Job SummaryProvide general assistance to our patients in any service and complete any process or application for them to receive benefits through the local resources and the Government Assistance Programs.Essential Duties & ResponsibilitiesAssist patients/visitors with the completion of the applications for SNAP, Medicaid, Medicare Saving Program, or any other Government benefit program, based on the information they provide and with the submission of verifications and general information regarding the application process.Assist patients/visitors to make any call connected to benefits and provide printer, fax, and copy machine for any application related document.Help patients/visitors to activate or request their Electronic Benefits Transfer card (EBT)Assist patients with the application of Medicare, Supplemental Security Income (SSI), Social Security card, change of status or any other benefit related to Social Security Administration (SSA).Assist patients complete the application for the Naturalization process and follow up until the end of the process.Complete the application of the American Passport and make the appointment to the corresponding office to compete the process.Assist patients with the application of Government cells phones, affordable High-speed low-cost Internet, Home Energy Assistance program (LIHEAP), Housing, Section 8, or any other public assistance benefit.Assist patients with the application for the Special Transportation Services, Bus pass application or Disabled Person Parking Permit.Participate in the department monthly meetings and/or services trainings to maintain the standards certificationsHandles any complaints from patients and/or family members as appropriate.Complete all process and handle all confidential information in an honest, ethical, and professional manner.Ensures that all HIPAA standards are met.Additional Duties & ResponsibilitiesEnsure compliance with all company policiesComplete annual trainings related to Social Services and those required by companySupervisory ResponsibilitiesNo supervisory responsibilitiesEducation & ExperienceMust have High School diploma or equivalent or years of relevant experience.Previous experience in customer service.Previous experience with any Government assistance programs and/or Immigration Services.Microsoft Office programs experience including Excel.Education RequirementsRequired/PreferredEducation LevelDisciplineRequiredHigh SchoolKnowledge, Skills & ProficienciesSkill in operating personal computer, phones and copy and fax machine.Ability to read, write and speak in English, Spanish and/or Creole languages helpful, and to write routine reportsSkill in oral communication and customer service. Ability to speak effectively and to communicate with employees, patients, and other individuals in a professional and courteous manner.Ability to solve practical problems and deal with situations where only limited standardization existAbility to pay close attention to detail, interpret a variety of instructions, follow up process, meets deadlines and to ensure accuracy of reports and data.Physical RequirementsThis position works under usual office conditions. The employee is required to work at a personal computer as well as be on the phone for extended periods of time.
