Provides outreach services in the community and in the clinics in an effort to improve patient care and outcomes.
- Registers and/or qualifies community members and clients for appropriate health center and community services; checks to ensure services are received and adjusts service arrangements as necessary.
- Convey the purposes and services of the program to patients and community partners.
- Conduct intake interviews with patients and provide referrals for services to community agencies as appropriate.
- Helps patients access resources including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible
- Provides information to community members concerning AHC Health programs. Participates in outreach and conducts follow-up client health visits when necessary.
- Coach patients in effective management of their chronic health conditions and self-care
- Assist patient in understanding care plans and instructions
- Follow-up with health management/care plans with both patients and providers.
- Arrange or assist patients in connecting with transportation resources.
- Establish positive, supportive relationships with patients and provide feedback.
- Effectively work with people (staff, patients, providers, agencies, etc) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.
- Communication and Health Information Entered into Record
- Enters appropriate treatment information into patient record in a timely fashion.
- Communicates patient's health status to provider to insure adequate provision of care.
- Accurately documents activities on patient health record and other reporting forms.
- Progress notes entered into patient health record to insure appropriate ongoing care.
- Conduct Emergency Department follow-up outreach by reviewing discharge documentation, assisting with care coordination, and educating patients on same-day appointments, after-hours services, and on-call provider resources to support appropriate healthcare utilization.
- Coordinate and maintain the sites Food Bank of Eastern Michigan (FBEM) pantry, including inventory management, ordering, and connecting eligible patients with available food resources.
- Continuously expands knowledge and understanding of community resources, services and programs provided.
- Use of Electronic Health Records
- Word & Excel
- Possesses a current State of Michigan driver's license and valid automobile insurance.
- Possesses a current CHW certification, if not, will be required to obtain one.
