As the Director of Social Work , you will play an important leadership role in supporting the psychosocial well-being of our residents. You will work closely with residents, families, the interdisciplinary care team, and community resources to ensure residents receive the support they need throughout their stay.

What You’ll Do

• Direct and oversee the facility’s social services program in accordance with federal and state regulations and facility policies

• Complete the Social Services portions of the MDS, Resident Assessment Protocols, care plans, progress notes, social services assessments, and other required documentation accurately and timely

• Assess residents’ psychosocial needs and develop appropriate interventions and support plans

• Assist residents and families with social, emotional, adjustment, and family-related concerns

• Participate in interdisciplinary care planning and identify residents’ social and emotional needs

• Maintain accurate and timely progress notes documenting residents’ progress and adjustment to the facility

• Make referrals to appropriate community agencies and support services and maintain documentation of referrals

• Participate in discharge planning and help residents and families prepare for transitions

• Serve as a resource for residents, families, legal representatives, and the community regarding facility services and resident needs

• Visit residents regularly and provide support, advocacy, education, and assistance with accessing appropriate resources

• Educate residents and legal representatives regarding Resident Rights and facility services

• Coordinate the logistics and content of monthly Family Council Meetings

• Coordinate resident room changes and assist with related communication

• Contact residents and legal representatives to encourage participation in care plan meetings

• Maintain ongoing communication with residents, legal representatives, and approved family members regarding resident care and needs

• Work with the Activities Coordinator to support residents who may lack family involvement or personal belongings

• Collaborate with the facility’s mental health provider to identify psychosocial needs, coordinate referrals, and ensure appropriate services are provided

• Promote positive working relationships with administration, physicians, medical staff, consultants, residents, families, volunteers, community agencies, and other members of the interdisciplinary team

• Assist with community education regarding aging, Resident Rights, facility services, and related topics

• Orient new residents to the long-term care environment and help facilitate a successful adjustment to the facility

What We’re Looking For

• Bachelor’s degree in Social Work or a bachelor’s degree in a related human services field, including sociology, special education, rehabilitation counseling, psychology, or a related discipline

• Minimum of one year of supervised social work experience in a healthcare setting working directly with individuals

• Previous social work experience in a healthcare or long-term care setting preferred

• Strong verbal and written communication skills in English

• High level of professionalism and ability to maintain confidentiality

• Strong organizational and interpersonal skills

• Ability to communicate effectively with residents, families, staff, and all levels of the organization

• Ability to work collaboratively as part of an interdisciplinary team

• Compassionate, resident-focused approach with strong customer service skills

• Ability to manage multiple priorities and work effectively in a fast-paced environment
