Client Care Coordinator

Community Bridges, Inc. (CBI) is an integrated behavioral health agency serving Arizona with residential, outpatient, and crisis services for individuals experiencing opioid use disorder, homelessness, and mental illness. Job Summary The Client Care Coordinator is a site‑based position at a shelter or temporary lodging site. Core responsibilities include managing a housing caseload, coordinating care for individuals and families experiencing homelessness, providing support services to end homelessness, and ensuring coordination with primary care and behavioral health providers. The Coordinator works with a multidisciplinary team that includes patient care techs, registered nurses, counselors, and clinical leads. Qualifications High school diploma or GED required; associate degree or higher in behavioral health preferred. Minimum six months recovery from substance use and/or mental health disorders. Minimum six months of paid related work experience in behavioral or medical crisis situations. Preferred 1–3 years of full‑time health‑care related work experience. Valid identification.
