This is done by partnering with senior living communities to coordinate and manage clinical gaps for residents to improve these community’s core business drivers. We deploy our own team of clinical experts both in-person and virtually paired with technology to provide instant leverage and relief for the community’s staff and fill gaps in care not always met by existing providers in the community. We honor residents’ existing care team including primary care via wraparound services to give residents the high-quality care and support they deserve, on their terms. Position Overview: Savoy Life is seeking a dedicated Resident Care Coordinator (RCC) to provide clinical and operations support within senior living communities in and around Spokane, Washington. The Resident Care Coordinator will work closely with an assigned Savoy Life RN as well as on-site senior living community teams to improve resident outcomes as well as a virtual provider. Key Responsibilities: Care Delivery Back Office Support Schedule various patient visits in the EHR to optimize the RN and APP’s time while coordinating with patient and caregiver needs Answer inbound calls, text messages, and voicemails in a professional and timely manner Monitor and manage inbound and outbound faxes Enter data, scan files, and print files as needed to support patient care and maintain updated historical records in the EHR in an organized and timely manner Identify and manage health plan insurance for residents Care Coordination Support Support the RN and Advance Practice Provider (APP) by coordinating patient care to drive patient outcomes Work with the local Savoy team to collaborate with other community resources that aid in Savoy’s goals of supporting patient care Manage facilitated telehealth visits with Savoy’s virtual provider Support the ongoing care with direct RCC led resident visits Manage communications and foster positive relationships with patients as well as others involved including caregivers, families, primary care providers, specialists, pharmacies, and senior living community staff Drive engagement with eligible residents, POAs, and families through education on Savoy’s offering to get consent forms signed Qualifications: High School Diploma / GED (or higher) 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience Experience in geriatric care or senior living community is highly desirable Must possess a valid and current driver’s license Excellent electronic medical record (EMR) documentation skills and general comfort leveraging various technologies to support day to day work Ability to juggle many tasks and switch between visiting with residents and coordinating behind the scenes Purpose-driven with a positive attitude to contribute towards a strong culture and passionate about improving senior living Flexibility to adapt to a dynamic and growing startup organization as the role and company evolve 100% employer paid, comprehensive health care including medical, dental, and vision for you and your family Paid maternity and paternity for 14 weeks at employees' normal pay Unlimited PTO, with management approval Opportunities for professional development and continued learning Optional 401K, FSA, and equity incentives available Hourly Rate: $25-30 Mission We make healthcare work for seniors and their caregivers.
