Community Support Specialist

As a member of the Federation of Catholic Charities, we serve the metropolitan and surrounding St. Louis area through four core programs: prevention, education, treatment, and housing. Job Summary Catholic Charities of the Archdiocese of St. Louis, Queen of Peace Center, is seeking a Community Support Specialist to provide person-centered rehabilitation and recovery support services to individuals with substance use and behavioral health needs. Working collaboratively with clients and the treatment team, this role helps individuals develop and achieve recovery goals, build independent living skills, access community resources, and strengthen natural support systems. The Community Support Specialist coordinates services, provides education and coaching, assists with crisis intervention and discharge planning, and supports clients in areas such as housing, employment, education, wellness, and daily living skills. Through a recovery-oriented approach, this position empowers individuals to achieve greater independence, maintain sobriety, and improve their overall quality of life while living successfully in the community. Job Responsibilities Partner with clients to develop individualized recovery and treatment goals by identifying strengths, needs, skills, resources, supports, and barriers to recovery. Assist in the development, implementation, monitoring, and periodic revision of treatment and rehabilitation plans. Support clients in building and maintaining natural supports and community connections that promote long-term recovery and independence. Provide rehabilitation services that help clients restore and develop life skills, coping strategies, healthy lifestyle habits, and resources impacted by substance use or behavioral health challenges. Collaborate with interdisciplinary team members to assess client needs and coordinate services related to employment, education, housing, nutrition, meal planning, budgeting, shopping, and other activities of daily living. Deliver individual and group education, coaching, and experiential training in life skills, resource acquisition, recovery management, and wellness. Link clients to community resources and services, assist with referrals, and advocate for access to appropriate, high-quality care and support services. Coordinate and monitor services provided by external agencies to ensure continuity of care, effective communication, and achievement of treatment goals. Respond to client crises by identifying needs, coordinating resources, and facilitating access to appropriate services and supports. Conduct outreach and re-engagement efforts with clients who miss scheduled appointments to support continued participation in treatment and recovery. Assist clients with discharge planning and transition to appropriate ongoing supports and services. Maintain accurate documentation and consistently meet productivity and billable service expectations. Transport clients, when appropriate and authorized, using a personal vehicle in accordance with organizational policies. Participate in required evening, weekend, and team coverage schedules. Job Requirements Bachelor's degree in Social Work, Counseling, Psychology, Human Services, Behavioral Health, or a closely related field; or a combination of post-secondary education and qualifying behavioral health, social services, or human services experience totaling four (4) years; or an Associate of Applied Science (A.A.S.) degree in Behavioral Health Support from an accredited institution; or Four (4) years of qualifying experience in behavioral health, social services, substance use treatment, case management, rehabilitation services, or a related human services field. Valid driver's license (MO residents require Class E), personal vehicle, and current automobile liability insurance that meets state minimum requirements.
