JOB DESCRIPTION TITLE: Lead Case Manager REPORTS TO: Lead Substance Use Counselor EMPLOYMENT STATUS: Regular, full-time CLASSIFICATION: Hourly LOCATION: Residential Treatment Big Lake AK FLSA STATUS: Non-exempt

GENERAL FUNCTIONS:

The Lead Case Manager plays a key role on the My House team, providing both therapeutic support and case management for youth in our 3.1 residential treatment program Jeff's Path. This position is responsible for developing client-driven treatment goals and coordinating access to services that support long-term success. Working in collaboration with Substance Use Counselor, Peer Support Specialist, 9 Star, and community partners, the Lead Case Manager helps reduce barriers and build support networks. Duties include coordinating care in areas such as housing, employment, healthcare, education, mental health services, and recovery.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Deliver group counseling using evidence-based practices.

Address issues including substance use, mental health, trauma, and co-occurring disorders.

Maintain accurate, timely, and confidential client records in AKAIMS in accordance with Alaska state standards. Utilizing DAP (Data, Assessment, Plan) notes format for documentation.

Coordinate services for clients, including referrals for mental health, medical, dental and other ancillary services utilizing partner providers.

Assist clients in accessing resources and advocating for their needs.

Work closely with Nine Star for a hand up system that trains and empowers homeless youth.

Coordinate services with Public Health and other medical providers that services for reproductive health, screening for STD/HIV and HEP-C and pregnancy, and birth control.

Work with family members, the court system, social service agencies, hospitals, and other service providers to offer wraparound services for homeless youth.

Conduct case staffings, provide feedback during case management and staff meetings.

Communicate effectively and be part of a team of staff professionals working together to support and encourage youth to set and accomplish goals, establish healthy boundaries and become productive.

Maintain a professional posture with clients, community members and partners at all times, using appropriate language and positive feedback designed to offer constructive growth that benefits the overall mission.

Represent MY House in a positive and proactive manner at all times.

Other duties and tasks as negotiated.

MINIMUM REQUIREMENTS:

Associates Degree in Social work, Psychology, Human Services, Counseling, or Substance Abuse/Addiction Counseling. Relevant experience may substitute for educational requirement on a year for year basis.

Two (2) years providing case management services.

Strong understanding of community resources and referral agencies

If in recovery must be in remission for two (2) year and working a program of recovery

CPR/First Aid/AED certified or able to obtain within 90 days of hire

Continued employment is contingent upon receipt of a satisfactory state and federal background check, approved National Provider Identifier Standard (NPI) status, and state Approval as a Qualified Addictions Professional (QAP) or a Provisional (QAP)

Must have a valid Alaska Driver's License and insurable under My House vehicle

PREFERRED REQUIREMENTS:

Bachelors Degree in Social work, Psychology, Human Services, Counseling, or Substance Abuse/Addiction Counseling. Relevant experience may substitute for educational requirement on a year for year basis.

Alaska Chemical Dependency Counselor Certification I (CDC I)

Working knowledge of Motivational Interviewing and Trauma-Informed Care

Current CPR/First Aid/AED certified

Current NPI and QAP

SKILLS/COMPETENCIES

Demonstrate the ability to respond to crisis situations and emergencies

Working knowledge of housing resources, rapid rehousing, permanent supportive housing and transitional housing.

Ability to deescalate agitated clients

Ability to maintain healthy boundaries with clients

Demonstrate flexibility, adaptability, and initiative

Working knowledge of health care portability act (42CFR and HIPPA)

Demonstrate positive work ethic

Ability to communicate proficiently verbally and in writing
