Physical Therapist (PT) – Home Health Case Manager Location: Chicago – South Side (servicing ZIP codes 60620, 60628, 60643 and surrounding areas) Schedule: Day shift with rotating weekends and holidays Sign-On Bonus: Eligible for up to $10,000 sign-on bonus for external candidates.
