Field-Based Physical Therapist - Home Health Case Manager

Adaptive Home Health is seeking a Field Based Physical Therapist (PT) to manage a full caseload delivering in-home skilled PT visits across Texas. You will assess, treat, and progress patients toward functional mobility goals, with a focus on gait, balance, transfers, and home safety. You will collaborate with a multidisciplinary team, document thoroughly in HCHB, and participate in scheduling and routing improvements as the company rolls out new tech tools. #J-18808-Ljbffr Home Health
