Field-Based Health Pathway Case Manager

The Salvation Army Ferguson Community Empowerment Center is seeking a full-time Pathway to Health Case Manager to recruit, enroll, and apply interventions for clients in Pathway of Hope and Pathway to Health. The role is community-based with frequent local travel in Ferguson, MO. Responsibilities include pre-screening, intake, goal setting, and regular home visits, with documentation in SIMS & HMIS databases.
