Job Title: Case Manager Profession: Registered Nurse Specialty: Care Coordination Duration: 12 weeks Shift: Day Hours per Shift: 8 hours Experience: 2 years required in case management in a facility with a minimum of 200 beds License: Required Certifications: ACM / CCM, BSN, ACLS Must-Have: - Experience with EPIC and Allscripts preferred - Previous case management experience at a facility with a capacity of 1000 beds or more Description: This is a 12-week position for a Case Manager. The position operates Monday through Friday for 40 hours per week. Case managers work either 7:00 AM to 3:30 PM or 8:00 AM to 4:30 PM based on unit needs. Case managers cover various units except for specific specialty areas. Responsibilities include completing assessments on all admitted patients. Attendance at interdisciplinary rounds is required. Caseload includes initiating referrals for home health, durable medical equipment, skilled nursing facilities, and other care transitions. The average daily census for the facility exceeds 1000 hospital beds. Candidates should have a minimum of 2 years of experience in case management, with a preference for applicants with experience in larger facilities. Academic experience is also preferred. Travelers must float within the service line, as needed. This role requires adherence to specific compliance deadlines and policies for submission and onboarding. All travelers must complete relevant certifications and labs within designated timeframes. No new requests for time off are permitted after submission of the application.
