Part Time Social Worker - Eddy Visiting Nurse & Rehab

Overview Part‑time Social Worker to manage a caseload of patients requiring social work support through in‑home visits and virtual/phone‑based services. Responsibilities Work with an interdisciplinary care team of nurses, therapists, and aides. Conduct comprehensive assessment of patient/family needs. Connect individuals to community resources needed to remain in home or transition to a higher level of care. Schedule and manage caseload with a flexible schedule, starting and ending days from home. Participate in the transition from inpatient to community (home health) care. Qualifications NYS Social Work Licensure. Minimum 1 year of relevant experience in home care, the medical field, or work with seniors. Benefits & Compensation Pay range: $30.00 – $43.08 per hour, based on experience, skills, education, and location. Flexible work schedule. Orientation to home health care and social work supervision. Supportive work environment with computer training and assistance obtaining insurance authorizations.
