Nurse Practitioner

Job Summary Advanced Practice Providers (APP) are licensed and board‑certified nurse practitioners or physician’s assistants assigned to Longevity Health Plan members to execute the Institutional Special Needs Plan Model of Care. APPs manage a panel of nursing home level care members in partnership with primary care teams, conduct comprehensive assessments, develop individualized care plans, attend facility care team meetings, communicate with families and care teams, and document patient status and diagnoses. APPs operate under the support of an APP Clinical Manager, Market Director of Clinical Services, and local medical director. Key Tasks and Responsibilities Provides collaborative primary care and services to a panel of Longevity Health Plan members, delivering the Model of Care and treating in the most appropriate setting. Executes advanced care planning, promotes the treating‑in‑place model, facilitates clinical operations functions such as prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and membership growth. Acts as a trusted advisor to members, families, nursing home staff, and advocates for member needs. Delivers frequent communication with PCPs, families, and care teams. Supports overall market goals and balances demand with team support needs. Promotes Longevity Health Plan among stakeholders. Operates autonomously and collaborates with other team members. Establishes meaningful relationships and functions effectively in nursing home environments. Performs other related duties as assigned. Supervisory Responsibilities There are no supervisory responsibilities for this position. Credentials & Coverage State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner OR state unrestricted licensed Physician’s Assistant NCCPA board certified. DEA License required. Active national provider number (NPI) and active Medicare provider number. Eligible to participate in the Medicare program as a provider and registered. Valid state driver’s license with a good driving record and proof of automobile insurance. Auto liability insurance declaration page with minimum limits of $100,000 bodily injury/$300,000 property damage and $100,000 for property damage with a combined single limit of $300,000. Proof of coverage provided annually. Education and Training Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program. Minimum 6 months experience as a Nurse Practitioner or Physician’s Assistant with experience in geriatric/hospice or palliative care populations; or licensed new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for 6 months and completing all competencies for independent practice as an LHP APP. Experience working in a nursing home environment is strongly desired. Computer/technology proficiency required. Ability to engage and communicate effectively with physicians, patients, family members, and care teams. Self‑directed but functions well in a team environment. Excellent clinical skills—assessment, diagnosis, treatment, prescribing for chronically ill populations. Demonstrated critical thinking, organization, and prioritization skills.
