Seven Healthcare are seeking an experienced Physical Therapist for a rewarding contract role in Ashland, KY. This travel assignment focuses on home health physical therapy, providing care across various counties including Boyd, Greenup, Lawrence, Carter, and Elliot.

- Perform physical therapy evaluation and treatment based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, medical history, precautions and contraindications
- Establish measurable goals and develop treatment plans
- Regularly re-assess treatment effectiveness and modify goals or treatment as needed
- Communicate patient needs and progress to the treatment team and family members
- Consult with medical providers on patient evaluation, treatment progress, and discharge
- Recommend and assist in obtaining special adaptive equipment
- Maintain and submit required documentation in a timely manner
- Comply with the Company's Code of Business Conduct and applicable state/federal laws
- Supervise Physical Therapist Assistants and Aides in patient care activities
- Participate in staff development programs and meet continuing education requirements
- Kentucky Physical Therapist Licensure or Compact Licensure accepted
- 1 year of home health physical therapy experience
- Previous OASIS experience
- Experience with home care home base
