RN Case Manager - Acute Care Schedule Days, M-F, 7:30 AM - 4:00 PM; No call, no holidays, no weekends Job Description As an RN Case Manager in Acute Care, you will be responsible for assessing discharge needs, addressing social determinants of health, and planning and referring for discharge of Acute Medical and Surgical patients. Responsibilities Conduct comprehensive assessments of discharge needs for Acute Medical and Surgical patients. Identify and address social determinants of health impacting patient care and discharge. Develop and implement discharge plans tailored to individual patient needs. Coordinate referrals to appropriate post-acute services and resources. Manage a typical caseload of 20 patients a day.
