Assignment Details

Contract Length: 13 weeks; Guaranteed Hours: 40

Requirements

3+ years of Acute Care Case Management experience
Must reside outside a 60-mile radius

Responsibilities

Assessment of discharge needs for acute medical and surgical patients
Evaluation of social determinants of health
Discharge planning and referrals
Manage a typical caseload of 20 patients per day
