Hospice RN

Hartford Healthcare is seeking a compassionate Hospice RN to provide expert end-of-life nursing care to patients in their homes and facilities across Connecticut. In this role, you will conduct comprehensive assessments, develop and manage individualized care plans, administer medications and treatments, and coordinate services with physicians, social workers, and other interdisciplinary team members. You will educate and support patients and families on symptom management and comfort measures, maintain detailed clinical documentation, and participate in case conferences while contributing to a culture of compassion, teamwork, and clinical excellence.
