Digital Health Navigator Location: US – Hybrid Requisition ID: 2026-38984 Position Category: Hospital/Clinic Support Job Type: AFSCME union represented Position Type: Limited Duration Full-Time Posting Department: Office of Care Transformation and Innovation Posting Salary Range: $26.90 - $36.35 per hour (offer based on experience, education and internal equity) Posting FTE: 1.00 HR Mission: Healthcare Drug Testable: Yes LinkedIn Job Code: #LI-BMN Department Overview The Digital Health Navigator, working as a Community Health Worker, partners with teams, staff and clinics across OHSU Health to improve access to digital health for Oregonians and SW Washingtonians. There are three focal points for this role: Deliver culturally appropriate digital health navigation support directly to patients, building comfort, confidence and skills with digital health technology. Focus on supporting patients during virtual/video outpatient visits via phone-based support. Emphasizes relationship‑building and coaching‑style approach. Contribute subject matter expertise to the Oregon ECHO Network, develop curriculum, participate in program sessions (trainings), possibly deliver components, and provide technical assistance to community‑based organizations, including support for a Community of Practice on a monthly basis. Develop and deliver culturally congruent, trauma‑informed health education classes, presentations and written materials. Build systems and services encouraging patient engagement and health behavior change such as community interventions, home visits, and family system interventions. The Digital Health Navigator demonstrates strong verbal and written communication skills, is self‑directed, and builds trusting relationships quickly. Fluency in Spanish is preferred to communicate with patients in Spanish (verbal and written). The incumbent is efficient and effective in assessing and delivering direct patient care, contributes to training and capacity‑building, and is innovative, creative and flexible. This is a grant‑funded position for approximately 16‑19 months (grant funding ends October 31, 2027). Function / Duties of Position Direct Patient Services and Documentation Respond to requests for digital health navigation support from care team members and/or patient self‑referrals. Deliver culturally appropriate digital health navigation support directly to patients, helping them participate in virtual visits through phone support. Build patient comfort, confidence and skills with digital health technology using relationship‑building and coaching approach. Support patients in successfully completing virtual outpatient visits. Call patients without a referral to offer support if desired. Apply knowledge of digital health tools and how they can be used by patients with diverse backgrounds (e.g., mother/baby, substance use, mental health, senior services, disability, basic needs, chronic disease management). Document services in the Electronic Health Record, following HIPAA guidelines and existing workflows. Maintain availability through standard departmental communication channels (phone, pager, email). Maintain professional boundaries as part of the care team. Collaborate with the digital health navigation team to enhance and adjust direct patient service. Oregon ECHO Training and Capacity‑Building Contribute to curriculum development, sharing subject matter expertise on digital health navigation. Participate in planning meetings, program sessions (trainings) for community‑based organizations and possibly deliver training components. Provide technical assistance post‑training to participating CBOs and facilitate ongoing shared practices and learnings. Health Education / Promotion Develop and deliver culturally congruent, trauma‑informed health education classes, presentations and written materials for various audiences. Develop systems and services encouraging patient engagement and increasing health behavior change, including community interventions, home visits, and family system interventions. Required Qualifications High school diploma or equivalent. Minimum 2 years of relevant work experience in a mission‑driven organization, with at least one year working with underserved populations having complex medical and social needs. Preferred experience with community outreach services providing health information, advocacy, social support, and engagement with the healthcare system. Certification as a Traditional Health Worker (THW) required. Types include Peer Support Specialist, Peer Wellness Specialist, Personal Health Navigator, Community Health Worker. Fluency in written and spoken English; excellent telephone communication skills. Valid motor vehicle driver license in good standing, or ability to access transportation to meet the position’s requirements. BLS certification from AHA required (new hires receive training during orientation). Pass a Spanish speaking and listening assessment (see Spanish speaking and writing skills that meet OHSU's requirements for patient verbal and written communications in Spanish are also preferred. Job Related Knowledge, Skills and Abilities Intermediate proficiency in Microsoft Word, Excel, and Outlook. Comfort using websites, mobile apps, and other computer or smartphone‑based platforms; interest in helping others learn to use them. Understanding of social determinants of health and their role in healthcare. Consistently, efficiently and accurately document service provision. Work independently using critical thinking, problem‑solving and time‑management skills. Skill in developing and maintaining professional relationships quickly. Plan and deliver interactive health education classes and presentations for various groups. Demonstrated understanding of trauma‑informed and patient‑centered care or willingness to learn. Interpret and follow policies and procedures. Preferred Qualifications Bachelor's degree. Experience working in a medical service delivery system or similar setting. Experience delivering trainings, health promotion/education, or providing technical assistance. Experience working with an electronic health record; EPIC platform preferred. Interest and experience in program development, improvement, and assessment. Spanish‑speaking language skills that meet OHSU's requirements for patient verbal and written communications in Spanish. Additional Details Frequent interruptions and frequent change with fluctuations in workload. May be exposed to body fluids, tissues, possible radiation or infectious processes. Subject to computer work for several hours, as needed. Use of office equipment including computer, smartphone, and pager.
